Want to become a patient?

We're currently accepting new patients on a waitlist, while prioritizing patients based on need. Patients enrolled with us are seen by any member of our compassionate and collaborative medical team.

Step 1:

Submit intake form

The intake form must be submitted for each member of your family intending to join the practice. The forms are not used to screen your admissibility: all individuals with a valid Ontario health card are eligible to enrol in our practice.

Step 2:

Allow for processing

Due to high demand for our family doctors in Caledonia, we have a waitlist. Individuals and families are rostered on a first-come-first-served basis while prioritizing based on need.

Step 3:

Book your Meet & Greet

Soon a member of our team will call you to book a Meet & Greet appointment. This is your orientation and opportunity to meet our healthcare team. We'll also update your health record and schedule any necessary future care appointments.

  • What about my medical history?

    When we are ready to enrol you as a patient, and only with your permission, we will request a copy of your health records from your former physician.
